- use by physiotherapists in this practice when consulting with you
- for communicating relevant information with other treating physiotherapists, general practitioners, specialists or allied health professionals
- followup, reminder or recall notices
- accounting, Medicare, Health Insurance procedures
- quality assurance activities e.g accreditation outcomes
- disease notification required by law
- legal disclosure as required by a court of law
- research purposes, but only once your information has been de-identified
By law you are entitled to have access to the information this practice holds about you. The practice may need to deny access to information in accordance with the exemptions contained in the APP Act. If you have any concerns or wish to restrict access please discuss this with staff.
